CD-ROM Drive Icon Disappears from File Manager

SYMPTOMS

The CD-ROM drive icon may disappear from the File Manager window and Event Viewer shows the following error:

   The following boot-start or system-start driver(s) failed to load:

      aha154x

This error does not allow the CD-ROM drive to be visible in File Manager.

This error may occur on a previously working CD-ROM drive after you install a new service or a software upgrade (such as installing a U.S. Service Pack).

WORKAROUND

To work around this problem, shut off your computer, and then restart Windows NT or Windows NT Advanced Server.

STATUS

Microsoft has confirmed this to be a problem in Windows NT and Windows NT Advanced Server version 3.1. We are researching this problem and will post new information here in the Microsoft Knowledge Base as it becomes available.
